Download the registration form here, fill it up and email to congress@apdc2011.org. To confirm your registration, please deposit your payments to BANK OF THE PHILIPPINE ISLANDS (BPI) Account Name: PDA-APDC Manila, Account No. 3981-0025-97. Then photocopy the deposit slip, put your name on it and fax it to +632.659.1509 or send it by email to congress@apdc2011.org or ladejesus53@yahoo.com. The registration fee entitles the registered participant to the following: Access to all lectures in the Scientific Program, Dental Trade Exhibition, Opening Ceremony, Welcome Reception and Award Presentation and Closing Ceremony; Receive a set of Congress documents (Offiical Program Booklet and Dental Trade Exhibition Catalogue), Congress bag, Congress badge and Certificate of Attendance; Receive purchased ticket(s) of social events; and Free gift(s).
